Return to School Documents
The Youth Brain Injury Program offers custom documents to meet the diverse needs of Alaska educational teams supporting youth returning to school following a brain injury. These documents were designed with inspiration from outside programs and technical assistance from the Oregon Center for Brain Injury Research and Training (CBIRT) to be in line with current best practices.

This document must be downloaded as a PDF for use.
We encourage all teams to use this document to improve consistency and effectiveness of temporary student support plans for all students returning to school following a brain injury.

Teacher Feedback Form
Teachers are encouraged to use this document to document and keep track of the accommodations being utilized in their classroom and provide feedback on the student's progress. The feedback document will assist the team in adjusting accommodations weekly or provide the team with suggestions if needed to move forward to a 504 or IEP.
Student Symptom Checklist
This document is unique and designed for the student to complete themselves, or initiate a conversation with the student about their symptoms, thus moving toward goals to resolve such symptoms and progress monitoring.
